These businesses will feature on the homepage and the services page of your website. Agreement Terms Please review and accept the terms and conditions of this agreement. A copy of this agreement will be emailed to you upon submission, one of our team will be in touch with you to start the process within 1 business day.Consent(Required)Agreement between Medical Together ("The Company") and "The Client" for Services at "The Centre" Appointment Cards 1.1 The Company shall provide information/appointment cards featuring sponsorship to The Client at no cost. 1.2 This agreement commences on the effective date. The Centre agrees to distribute these cards to their patients for a minimum of two years from the publication date. 1.3 The Company retains the right to delay printing until adequate sponsorship is secured. 1.4 The Centre agrees not to utilise an alternate sponsored appointment card. 1.5 Either party may terminate this agreement with written notice, effective 24 months after the current sponsorship term. 1.6 In the event of a change in ownership or control of the Centre, the Client must notify the Company, providing the date of the change. The Company and the proposed transferee or controlling entity will facilitate the transfer of this Agreement. Website 2.1 The Company shall provide The Centre with a professional website containing up-to-date information on medical center services, staff, and contact details. The website will feature click-to-call buttons, booking integration, and contact forms upon request. The website provided will be designed to be responsive on mobile and tablet devices and optimised for search engines. The website will be secured with an SSL certificate to enhance security and improve its ranking potential on Google. 2.2 The Centre must utilise the provided website as their primary online platform and exclusively refer to this website for the duration of the sponsorship cycle (24 months). 2.3 The website will display banners promoting local businesses that sponsor The Centre website. The banners/features appear under the headline “We also support our local services near the medical centre”. 2.4 The Company will ensure that the developed website aligns with the medical center's brand and identity within the confines of the selected template. The template images, logo, colours and typography can be adjusted to conform with the medical centre identity/brand. 2.5 If The Company fails to secure minimum website sponsorship in the first 3 months, The Client agrees to pay a monthly fee of $129.00 + GST for the full 24 month duration. Should The Company obtain minimum sponsorship during the 24 month cycle, the monthly fee will cease for the duration of the sponsorship cycle. 2.6 There is an option for a bespoke website design for a one-time fee of $2,000.00 + GST. Please advise if you wish to have a bespoke design for your website. 2.7 Minimum website sponsorship is deemed to be 3 sponsors featuring on the website. Should only 1-2 sponsors be obtained, the monthly fee - see 2.5 will apply. Website Updates 3.1 The Company shall grant The Client access to the content management system (CMS) for updating the website's content. Sponsored features must not be removed. 3.2 If requested, The Company will handle updates to content, The Client will be advised of any fees for updates prior to the updates being implemented. Website Ownership 4.1 The Company retains ownership of the website and its assets. The domain name belongs to The Client. 4.2 The Company will provide hosting for the website. In order to ensure the integrity of the sponsorship, the website must be hosted on the company’s server. Privacy 5.1 The Company will not share specific private website information with third parties, except for reporting total traffic and click rates on sponsored features. Termination 6.1 This agreement lasts for a minimum of 24 months from sponsorship commencement and cannot be terminated before sponsorship expires without compensation. 6.2 Notice of termination should be provided 3 months in advance and prior to the sponsorship expiration. Upon request of termination, this arrangement will only be terminated after all sponsorship has expired. Change of Ownership 7.1 If the Centre undergoes a change in ownership or control, The Client must inform the Company. The Company and the proposed transferee or controlling entity will facilitate the transfer of this Agreement. Representation 8.1 The Client grants The Company permission to act on behalf of The Centre throughout this agreement to facilitate the sponsorship process. Conflict Resolution 9.1 Before securing sponsorship, The Company employs rigorous processes to avoid conflicts of interest with in-house services or allied health providers. In the event of new services/providers joining after sponsorship is finalized, while efforts will be made to reposition sponsors, The Company retains the right to maintain sponsors on cards/websites for the sponsorship cycle's entirety. I have permission to act on behalf of the medical centre and agree to the terms provided.(Required)
